Telephone Crash Information
Exchange and Reporting Program
The circumstances surrounding your automobile crash meet existing criteria for reporting this incident by telephone at a later date and time if:
1. There were no injuries involved in the crash.
2. No vehicles involved in the crash required towing.
YOU MUST REPORT THIS CRASH WITHIN 10 DAYS
Your crash report can be filed by calling the Illinois State Police at 1-847-294-4400. Please follow the reporting instructions listed below.
1. Leave your name and a telephone number where you can be reached between the hours of 8 a.m. and 2 p.m., Monday through Friday. A call will be returned to you from the Crash Report Center.
2. Have the following information available for the return call from the Crash Report Center:
a. Your driver's license number
b. Vehicle insurance information
c. Vehicle information including make, model and year
d. Vehicle registration card
e. Vehicle license plate information
f. Vehicle identification number (VIN)
After filing you report, you will be mailed a copy of the face of the report containing driver and vehicle information you have provided. Additionally, you or your insurance company can obtain a complete copy of this report by sending your request along with a self addressed, stamped envelope and a check or money order for $5.00 made payable to the "Illinois State Police". Your request should included the date of the crash and the Crash Report Number. Requests should be mailed to:
